Shift notes
This guide is for a support worker. Write what happened in your own words. A signed note is locked; later corrections are dated additions. The participant can read the submitted note.
Before you start
Clock out and check the actual times. See Shifts.
Write on the native phone app
- After clocking out, tap Continue the note. You can also use Add, then Start a note, and choose an eligible finished shift.
- Check the participant and shift times. Use Adjust times if needed.
- Complete Location, NDIS support item, What happened on the shift, the participant's response and the handover.
- Under Plan goals this shift worked on, choose the relevant goals. If none apply, choose No applicable goal or Goals not added yet as appropriate.
- Check the save status. Save and close stops for now. Review and sign moves to the final check.
Tidy with AI is unavailable in this version. Do not expect it to change your words.
Sign on the native phone app
- Read Sign this note and the full note summary.
- Check the price and budget preview when available. If the times differ from the booking, correct them before signing.
- Type your full name exactly as your account records it.
- Tap Submit note.
- Check whether the result is Note submitted and locked or Note signed and queued.
A queued note is still waiting to sync. It is not proof that the participant can read it yet. Reconnect and check the final status before repeating submission.
Write and sign on the web
Open Shifts, choose the shift and select Create shift note. Check the agreed support code and actual times. Write what happened, the participant response and handover, and choose the goals supported or explain why none applies. Select Save note for review, read the saved words, then type your full legal name and select Sign saved note and prepare charges. Open Daily review to check the priced support before preparing a timesheet.
You can also continue text through Unfinished notes, then open the shift to review and sign. Signing preserves that version; it does not approve a participant’s timesheet.
If the draft changed on another device, reload and review the newer record. Do not overwrite it without checking your unsent words.
Correct a signed note
On the phone, open the signed note and select Add a dated note. Enter why you are adding it and what needs adding. Type your full name and select Save addendum. The original stays unchanged. A queued note must sync before an addendum can be saved.
If something goes wrong
- Choose why no goal is linked. Select a relevant goal or the appropriate no-goal reason.
- Saved on this phone. Your local draft is kept, but syncing may still be waiting. Read the save status before closing.
- Submission was not confirmed. Check the note's record and queue before retrying.
